This agreement between ____________________________ and Second Opinion Insurance Services is subject to the following terms and conditions: Fees The above referenced agent will pay fees to Second Opinion for services provided through its Mortality Manager ™ program. The fee for Mortality Manager services for cases that are placed with Non-Second Opinion carriers is $5,000.00 or 15% of the first year target premium and a 1% renewal for years 2-10, whichever is higher. A fee of $750.00 will be paid to Second Opinion when the case is not placed with any carrier. No fees are charged for a client who is uninsurable or is highly rated.
